What is a Child and Family Team (CFT)?
Uplift Family Services Child and Family Team Specialists facilitate meetings for families with open cases with Sacramento County Child Protective Services. The child/youth, family members, trusted adults, and caring professionals work together in achieving goals for positive change and improving child safety, permanency, and well-being.
- Supports a child or youth and families involved in the child welfare system.
- Works with the family to develop and support a shared vision and goals.
- Includes natural supports, as well as professionals involved with the family.
- The child/youth and family’s voice and choice are central to the Child and Family Team process.
Who can participate in a Child and Family Team?
At a minimum, the core CFT members include: the child/youth and family, the child Welfare Services social worker, and the behavioral Health Services provider. Other teams members can include caregivers/extended family, service providers, coaches/neighbors, tribal members, court Appointed Special Advocate (CASA), spiritual and faith-based supports, family and youth partners, and others who support the family.
How can I get a CFT referral?
For Uplift to facilitate a CFT, we must receive a referral from a Sacramento County Child Protective Services Social Worker. Families should contact their Sacramento County Social Worker and request a CFT.
